הגישה המודרנית אינה "פחות עצם בכל מחיר". לעיתים דווקא אוגמנטציה נרחבת היא הדרך היחידה להגיע למיקום שתל ולשיקום נכונים. החידוש הוא שהצלחת ההליך אינה נמדדת עוד במספר המילימטרים שנוספו לרכס. המטרה היא ליצור את כמות העצם הנדרשת לשיקום המתוכנן – לא יותר, אך גם לא פחות. במובן זה השתלת העצם עוברת מתפקיד של שחזור אנטומי אוטומטי לכלי הנבחר בהתאם לדרישות השיקום העתידי.
