מצד אחד, ומאידך קשובה ליכולות שלהם להכיל את המידע ולספוג אותו, מאפשרת לנהל דיאלוג פתוח, תוך שמירה על גבולות ברורים מתוך רצון להגן על הילד. יצירת תחושה של קירבה והעצמה של הקשרים בינן לבין הילדים, הייתה עבור האמהות מנוף להתמודדות, הסתגלות וצמיחה. סרטן מתקדם במצב בו המחלה מתקדמת, שגרת החיים עלולה להפגע באופן משמעותי. לעתים מדובר בתהליך ארוך ומתמשך שעלול להגיע למספר שנים, עם ירידה הדרגתית במצב הבריאותי, עליות ומורדות והסתגלות איטית במהלך הדרך. בעקבות ההחמרה במחלה יש צורך לארגן מחדש תפקידים שונים ותחומי אחריות. מסיבה זו, מצב של מחלה מתקדמת, מלווה לעתים בנטל על בני משפחה קרובים, בעיקר על בן הזוג. אלה נדרשים לסייע לחולה patients: relationships to coping and social support. ולהתמודד עם הידיעה על המוות שהולך וקרב. לו וחבריו [52] מנסים לבחון מה הם הגורמים המגנים, המסייעים למשפחות להתמודד עם נסיבות חיים קשות של מחלת סרטן מתקדמת. החוקרים מדגישים את מרכזיות המרכיב הבין-אישי, תחושת , היכולת )Relatedness( השייכות- מחוברות לתת ולקבל, והאמון שנוצר, או חלילה נשבר, במערכת היחסים. החוקרים מציינים כי עבור חולים עם מחלת סרטן מתקדמת, היכולת למצוא משמעות ומטרות בחיים נובעת מתחושת שייכות, הערכה ותפיסה עצמית, שהופרעה בשל הנטל והעומס שיוצרת מחלה. במחקר שכלל 872 זוגות של חולים עם מחלת סרטן מתקדמת מצאו החוקרים, שהחוויה של בן הזוג הבריא היתה, כצפוי, קשורה לחומרת המחלה, אבל גם לתחושת השייכות. ככל שתחושת הנטל על בן הזוג היתה קטנה יותר ובמקביל, המצוקה של החולה פחתה, כך עלתה תחושת השייכות/מחוברות אצל בן הזוג הבריא. 10. ממצאים אלה חוזרים ומדגישים את הצורך לטפל במערכת המשפחתית כולה, ולא רק בחולה, בעיקר כאשר מדובר במחלה מתקדמת המאיימת על החיים. סיכום אחד האתגרים העומדים בפני אנשי המקצוע מתחום הפסיכו-אונקולוגיה הוא הצורך לסייע לחולות ולבני משפחותיהן להתמודד עם הדואליות של חיים ומוות. אתגר זה מתעצם שבעתיים לאור העובדה כי חולות רבות שלקו בסרטן השד מתמודדות עם האתגר הפרדוקסאלי בו הן נדרשות לדבוק בחיים כאשר מצד אחד הן מודעות למצבן, ומאידך הן מתמודדות לעיתים - במצב של מחלה מתקדמת - עם מוות צפוי. היכולת לשאת את הפרדוקס הזה תלויה ביכולת לשמר Double( מידה מסוימת של מודעות כפולה לאפשרות של חיים, ולאפשרות )Awareness של מוות. 1.
